What should be done if the Ketubah is lost?

In case the Ketubah is lost, the couple must go to a certified rabbi to write them a "Ketubah D'Archsa". In such a Ketubah document, witnesses testify that a certain man and woman came before them and stated that their Ketubah document was lost, and they write a "Ketubah D'Archsa".
