Must one hear their own blessing when reciting it?
For any blessing that one recites, one should make it audible to one's own ears. However, after the fact, if one moved their lips but did not make it audible to their ears, they have fulfilled their obligation [Shulchan Aruch 185:1 and 206:3].
Nevertheless, if one is in a place with much noise and cannot hear their blessing, they fulfill their obligation as long as they raised their voice.
May a woman recite a blessing without covering her head?
It is forbidden to recite any blessing with an uncovered head, and there is no difference in this matter between a man and a woman. In our generation, the custom has been that unmarried women recite blessings without head coverings, and we allow them to maintain their custom [Yabia Omer 6:15].
